Azria Health is seeking a Regional Nurse Consultant to support Skilled Nursing Facilities in the Des Moines, IA area. The role anchors clinical systems, quality assurance, and staffing, with collaboration to Nursing leadership and Administrators.

Qualifications

  • RN license required.
  • 2 years of relevant nursing experience preferred.
  • High school diploma or equivalent preferred.

Responsibilities

  • Overall support of the nursing departments for multiple facilities.
  • Support for the Director of Nursing and Administrator at each facility.
  • Clinical Resource helps implement strong clinical systems for highest quality care.
  • Develop and implement quality assurance measurements and audits.
  • Assist in improving medical management processes across facilities and market.
  • Assist in budget preparation and monitor monthly nursing performance vs budget.
  • Schedule rounds to monitor quality and appropriateness of care.
  • Develop and implement staffing plans and nursing schedules reflecting resident needs.
  • Evaluate current and potential residents to determine appropriate care level.
  • Monitor QA processes to ensure regulatory/accreditation standards.
  • Serve as clinical resource to nursing administration teams at facilities.
  • Provide training to staff nurses to promote performance improvement.
  • Support during surveys including pre-survey preparation and post-survey plans.
  • Maintain communication with Medical Director to identify educational needs.

The Regional Nurse Consultant will support Skilled Nursing Facilities in the Des Moines, IA area.

  • Overall support of the nursing departments for multiple facilities. Responsible for supporting the Director of Nursing and Administrator at each facility.
  • The Clinical Resource helps provide input on, and support for, the implementation of strong clinical systems that result in the highest quality of care.
  • The Clinical Resource supports the development and implementation of quality assurance measurements and audits, and assists in improving medical management processes throughout the skilled nursing facilities and market by coordinating and engaging in activities to proactively promote implementation of evidence based best practices.
  • Supports the Director of Nursing for the provision of quality and appropriate resident / patient care that meets or exceeds company and regulatory standards
  • Assists in the preparation of annual budget for the facility. Monitors monthly performance of nursing services in relation to budget and intervenes as needed
  • Schedules and performs rounds to monitor and evaluate the quality and appropriateness of nursing care
  • Assists in Development and implementation of the written staffing plan and nursing schedule that reflects the needs of the resident and patient population
  • Evaluates current and potential residents and patients to determine and maintain living center’s ability to provide appropriate level of care
  • The Clinical Resource monitors, performs, and manages quality assurance processes including those supporting compliance to regulatory and/or accreditation standards
  • Function as a clinical resource to the nursing administration team at each facility
  • Provide training to staff nurses to promote performance improvement
  • Provide support during the survey process including pre-survey preparation and developing performance improvement plans post survey if necessary
  • Maintains regular and ongoing communication with the facility's Medical Director to identify educational needs for staff and provide quality care and services to meet the needs of residents.
  • Monitors daily Clinical Start-up meeting with the interdisciplinary team to review resident and patient status on a regular basis
  • Collects, reviews and analyzes clinical outcome data and determines trends. Brings identified concerns to the QA&A committee for development of appropriate plans of action at the facility and regional level

Provides Support for clinical aspects of state or federal government survey processes
