Job summary

CareMed is seeking a Medical Director for Skilled Nursing & Long-Term Care facilities to provide clinical leadership and medical oversight. You will collaborate with NPs, nursing leadership, and interdisciplinary teams to drive quality, safety, and regulatory compliance across multiple facilities.

The role emphasizes development of clinical best practices, program oversight, and performance improvement to optimize patient outcomes and reduce avoidable hospitalizations.

Qualifications

• MD or DO degree from an accredited institution.
• Board certification in Internal Medicine, Family Medicine, Geriatrics, or related specialty preferred.
• Active and unrestricted medical license in the state of practice.
• Experience in skilled nursing facilities, long-term care, post-acute care, or geriatrics preferred.
• Experience collaborating with nurse practitioners and interdisciplinary teams.
• Strong leadership, communication, and organizational skills.

Responsibilities

• Provide medical direction and oversight for resident care within skilled nursing and long-term care facilities.
• Establish and maintain clinical standards, protocols, and best practices.
• Support management of complex geriatric and chronic medical conditions.
• Participate in development of care plans and clinical pathways.
• Review quality metrics, hospitalization rates, infection control practices, and patient outcomes.
• Collaborate with nurse practitioners in resident care delivery.
• Provide mentorship and clinical guidance to advanced practice providers.
• Conduct regular case reviews and interdisciplinary discussions.
• Promote team-based, evidence-based care delivery.
• Ensure compliance with federal, state, and local regulations.
• Participate in surveys, audits, and quality assurance activities.
• Lead Quality Assurance and Performance Improvement (QAPI) initiatives.
• Work to improve patient satisfaction and outcomes.
