Continuum Healthcare Inc. is seeking a Regional MDS Coordinator to oversee 7 long-term care campuses across four states. The RNAC will ensure accurate MDS/RAI processes, coordinate with interdisciplinary teams, and stay current with regulatory changes to deliver high-quality resident care.

Qualifications

• Active RN license in good standing.
• Experience in long-term care settings is required.
• RAC-CT certification is preferred but not mandatory.
• Solid knowledge of MDS and RAI processes.
• Strong organizational and time-management abilities.
• Excellent communication and interpersonal skills.
• Proficiency with common computer applications.
• Understanding of quality measures and indicators.

Responsibilities

• Conduct comprehensive assessments of residents and care needs.
• Coordinate with interdisciplinary team to plan individualized care.
• Ensure timely MDS and RAI completion and accuracy.
• Collaborate with healthcare providers to implement care plans.
• Stay updated on changes to MDS/RAI regulations and guidelines.
• Provide training and support to staff on MDS/RAI processes.
• Communicate effectively with residents, families, and providers.
• Monitor Case Mix index and Quality Measures.
