Job description


Summary

The Regional Home Health Survey and Performance Improvement Coordinator reports to the Home Health Survey and Performance Improvement Manager and is responsible for facilitating overall agency survey readiness and successful survey outcomes.

Must be available to travel up to 50% within region (OH, MN, MI, MO, and IL.)



Responsibilities
  • Performs on-site survey readiness assessments at least annually for each assigned provider using designated
    protocols and tools.
  • Guides the agencies during state and Joint Commission surveys, as applicable, by remaining available during
    survey to navigate the team through the survey process.
  • Collaborates with agency leaders on the development of the required action plan, and monitors follow
    through with action plans/Evidence of Standards Compliance (ESC)|Measures of Success (MOS).
  • Maintains current knowledge of regulations affecting the agency and acts as a resource to agency leadership
    regarding Conditions of Participation, State Minimal Standards, Joint Commission, and state discipline specific
    practice acts.
  • Provides face-to-face exit conferences with Executive/Clinical Director or designee after each site visit to
    discuss survey findings
  • Collaborates with Regional Quality and PX Coordinator regarding the development of survey plan of correction
    including monitoring activities to consider existing improvement plans, combining activities to minimize any
    duplication of improvement activities.
  • Collaborates with agency leaders, RVPs, and Regional Quality Coordinators to ensure compliance with state,
    TJC, and survey readiness improvement plans and provides guidance to ensure mitigating factors have been
    successfully addressed to ensure ongoing regulatory compliance.
  • Performs inter-rater reliability audits on quality chart reviews performed by agency leaders during the mock survey process. Communicates findings with ED/CD and provides remediation to improve auditing accuracy.
  • Communicates remediation plan with Regional Quality Coordinator for follow-up.
  • Regularly communicates progress and concerns with the respective Regional Quality and Patient Experience
    Coordinator, Regional Vice President, and Survey Team Manager.
  • Adequately prepares for and actively participates in survey review calls with Survey Team Manager, as
    requested.
  • Performs other assigned duties timely and adequately.


Education and Experience
  • 3 years healthcare experience in home health quality required.
  • Bachelor's Degree in a clinical health care related field.
  • Associate Degree acceptable for RN candidates only.
  • Certification in HealthCare Quality is preferred, after first year of service.
  • Current Driver's License and vehicle insurance, and access to a dependable vehicle.
  • Understanding of CMS Conditions of Participation, Joint Commission Standards and state licensing standards.
  • Effective written and verbal communication skills.
  • Good interpersonal skills.
  • Fiscal responsibility and accountability.
  • Proficient computer skills with knowledge of Microsoft Word and Excel.
  • Goal-oriented and ability to affect change.
