Job summary

P3 Health Partners is seeking a Regional Quality Coordinator to manage quality operations and oversee special projects, supporting HEDIS and Pharmacy gap closures across affiliate groups. You will engage in education and outreach to patients for preventative care and adherence, coordinating with internal teams and external partners.

You will work under the market VP and National Quality Manager, traveling as needed and operating in a hybrid office setting.

Qualifications

• 2+ years experience in healthcare-related fields.
• Experience with Electronic Health Records (EHR) required.
• Medical terminology knowledge required; CPT II codes preferred.
• Medical Assistant certification or Pharmacy Tech/Radiology/Lab tech experience preferred.
• Experience with data mining is preferred.

Responsibilities

• Provides telephonic outreach to patients, providers, and pharmacies.
• Supports Quality Management through HEDIS, CMS Star Rating, and reporting.
• Participates in data collection from medical records and claims surveillance.
• Leads project management of Quality Fairs, Diabetic Days, and P3 events.
• Interacts with patients to close quality gaps and improve care outcomes.
