Job summary

Telligen is seeking a Quality Improvement Facilitator focused on Home and Community Based Services (HCBS) quality and compliance. You will evaluate HCBS waiver, habilitation, CNRS providers, and self-directed services to ensure adherence to state and federal rules and industry best practices.

This role requires residence in Iowa counties: Scott, Linn, Jones, Cedar, Jackson or Clinton, or surrounding areas. Travel within the region is expected as part of provider evaluations.

Qualifications

• Bachelor's degree in healthcare administration, social work, public health, or related field.
• 3–5 years HCBS or quality improvement experience.
• Knowledge of state and federal HCBS regulations and requirements.
• Strong analytical and problem-solving abilities.
• Excellent written and verbal communication skills.
• Willingness to travel within assigned region.
• Valid driver’s license and ability to travel within region.

Responsibilities

• Conduct quality evaluations of HCBS waiver, Habilitation, CNRS providers, and self-directed services.
• Assess provider compliance with state and federal regulations, policies, and procedures.
• Develop and implement corrective action plans with providers.
• Provide technical assistance and guidance to providers on HCBS topics.
• Document findings and maintain detailed records of provider evaluations.
• Analyze data and prepare reports on provider performance and compliance.
• Facilitate training sessions and educational workshops for providers.
• Collaborate with stakeholders to implement quality improvement initiatives.
• Monitor provider progress in meeting quality standards.
• Stay current with evolving HCBS regulations and best practices.
