Job summary

Centric Home Health and Hospice seeks a Regional Director of Clinical Services, a licensed RN responsible for ensuring high-quality patient care across home health regions. You will coordinate interdisciplinary care, supervise field staff, and ensure adherence to agency policies and regulatory standards.

This on-site role alternates between the Lawton and Norman offices in Oklahoma, requiring strong leadership, clinical expertise, and reliable transportation.

Qualifications

• Graduate of an accredited Nursing program (Diploma, ADN, or BSN).
• Current RN state license.
• Minimum 2 years of Home Health field RN experience within the last three years.
• Management experience preferred.
• Current CPR certification.
• Strong interpersonal and communication skills.
• Reliable transportation with valid driver’s license and auto liability insurance.

Responsibilities

• Provide leadership and supervision to field staff.
• Coordinate patient care with the interdisciplinary team, physicians, patients, and families.
• Conduct ongoing assessments and help develop patient plans of care.
• Ensure compliance with home health regulations and quality standards.
• Participate in the QAPI program and agency in-service training.
• Support staff competency evaluations and performance monitoring.
• Maintain accurate clinical documentation and support reimbursement requirements.
• Participate in on-call rotation as assigned.

About the Role
