  • Maintain accurate documentation and timely status updates to providers and patients.
  • Communicate any processing issues promptly to the care team.
  • Travel within facility and community as needed.

SUMMARY OF POSITION

The Referrals Coordinator facilitates the processing of patient referral orders originating from Vibrant Health providers to outside healthcare providers and/or facilities. This includes but is not limited to: insurance verification, data entry, obtaining necessary pre-authorizations and communicating with other medical facilities as needed.

ESSENTIAL JOB DUTIES
  • Closing the loop of care; from receipt of referral order upon origination to receipt of consult note, report, result, etc.
  • Coordinating all specialist referrals, appointments and pre-authorizations related to productivity thresholds and benchmarks (turn-around time).
  • Tracking referrals in accordance with our policy and procedures with status documented to completion.
  • Mandating contact with patient if referrals are not completed within one (1) month to determine reason for lack of follow up.
  • Documenting and communicating to initiating provider the reason for referrals not completed by patient.
  • Verifying eligibility, dates of coverage and benefits for services to which patients are being referred.
  • Gathering, sending and exchanging appropriate healthcare information to provider(s) receiving referral(s) in accordance with information security and privacy protocols, regulations and legal statutes.
  • Effectively and professionally communicating status of any authorization denial to patient and the care team.
  • Responding to inquiries related to referrals (internal and external sources).
  • Notifying patients of any processing issues regarding their referral.
  • Working in collaboration with care teams.
  • Developing and maintaining contact with specialty offices and diagnostic facilities.
  • Establishing and maintaining effective, professional working relationships with all internal and external customers including other healthcare offices and facilities.
  • Maintaining skills and training required to provide coverage of reception duties when needed.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
  • High School diploma or equivalent.
  • Two years of relevant work experience, preferably in a medical setting.
  • Proficiency in use of personal computers and Microsoft Office Suite (Word & Excel).
  • Two years of experience working with electronic medical records.

  • Hold current driver’s license and reliable transportation.
  • Travel to all parts of the facility and throughout the community.
PREFERRED KNOWLEDGE, SKILLS, AND ABILITIES
Prior experience working in a health care environment.
  • Bilingual fluency in written and oral English/Spanish.
