Job summary

CommonSpirit Health at Home is seeking a Referral Operations Coordinator to handle physician orders, manage referral documents, and maintain strong client and branch relationships. You will ensure timely documentation, support home health and hospice goals, and track physician-signature materials using enterprise systems.

Ideal candidates have at least two years in customer service and preferably medical office experience, with strong confidentiality and communication skills.

Qualifications

• High School diploma or equivalent.
• Minimum of two years customer service experience.
• Minimum of one year of medical office related experience preferred.
• Must possess the ability to maintain confidentiality of HIPAA protected health information that adheres to the Corporate Compliance Program.

Responsibilities

• Processing approved physician orders.
• Delivering and picking up documents that need physician signature for identified accounts.
• Effectively presenting and discussing products and services to convey quality and customer understanding.
• Promoting open flow of information and education to support home care and hospice goals.
• Completing and submitting required documentation accurately and timely.
• Developing and maintaining accurate files on each key referral source for client information and branch ties.
• Tracking documentation requiring physician signature using standard document management software and processes.

• Processing approved physician orders.
• Responsible for delivering and picking up documents that need to be signed by a physician for identified accounts.
• Effectively presents and discusses the products and services of the organization in a way that conveys an image of quality, integrity and superior understanding of customer requirements.
• Promotes an open flow of information and education in support of home care and/or hospice goals.
• Completes and submits all required documentation in an accurate and timely manner.
• Develops and maintains accurate files on each key referral source to provide the location with client information needed to build strong client and branch ties and deliver customer satisfaction.
• Tracking documentation requiring a physician’s signature using organization standard document management software and processes.
• Follows up on items needed in support.

Job Requirements

Candidates must reside in one of the approved remote worker states: AR, AZ, CO, FL, GA, IA, IL, IN, KY, MI, NC, ND, NE, NJ, OH, SC, TN, TX, UT, WI

• High School diploma or equivalent.
