Behavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic and talented Referral Management Clinician - PRN 12 hours Night Shifts that serves in managing electronic referrals, coordinating care with facilities, patients, and families, and conducting clinical reviews of medical records. Collaborating with healthcare providers to ensure safe patient placements and compliance with HR policies and HIPAA standards.

Qualifications
EDUCATION
Masters Degree in Social Work, Psychology, Counseling, Behavioral Health or related field.
LICENSURE/CERTIFICATION
Preferred Licensed in state, LMSW, LCSW, LPC. Current CPR, Vaccines, if required by a customer hospital.
EXPERIENCE
Preferred 1-3 years work related experience.
