Our service model focuses on the assessment, diagnosis, treatment, and disposition of your mental health patients. Based on the results of an assessment, we determine the most appropriate level of care and recommend treatment facilities. The tele-psychiatric assessment and consultation services we offer are designed to connect people with the care providers best equipped to address their mental health issues. Rapid response means that the patient will get to the most appropriate level of care faster and expedite their journey to wellness.

Behavioral Health Connections (BHC) in Tempe, AZ is seeking a dynamic and talented Referral Management Clinician - PRN 12 hours Day Shifts that serves in managing electronic referrals, coordinating care with facilities, patients, and families, and conducting clinical reviews of medical records. Collaborating with healthcare providers to ensure safe patient placements and compliance with HR policies and HIPAA standards.

Job Duties/Responsibilities:
  • Review assessment referrals received from various sources in collaboration with the RN and other clinicians, to assess patient needs.
  • Assist in coordinating patient care.
  • Maintain patient records, update databases, and ensure accurate documentation of patient information in approved EMR and other platforms.

Qualifications
EDUCATION

Masters Degree in Social Work, Psychology, Counseling, Behavioral Health or related field.

LICENSURE/CERTIFICATION

Preferred Licensed in state, LMSW, LCSW, LPC

Current CPR, Vaccines, if required by a customer hospital.

EXPERIENCE

Preferred 1-3 years work related experience
