Overview

El Camino Health Medical Network is currently seeking a talented Referral Coordinator to join our growing team!

Pay: $25.55- $32.00/hr

Location: Los Gatos, CA

Shift: M-F, 8am - 5pm

Responsibilities

The Referral Coordinator is responsible for supporting all elements of referral and authorization to include validating patient’s health plan eligibility and obtaining authorization for all services and procedures in office and external facilities. Responsibilities include contacting patients and scheduling into the Epic system, and contacting clinical staff to coordinate peer-to-peer calls between physicians and the health plan as needed.

The Referral Coordinator will work as an educator, healthcare resource, and patient advocate to ensure patient care is coordinated and managed appropriately. He/she will refer patients to external physicians/specialists, serve as a liaison to the physician, and coordinate care across the healthcare continuum.

Qualifications

• A minimum of 5 years of experience in healthcare (acute care or ambulatory care organization), preferably experience in referral coordination or utilization management
• Knowledge and experience with Electronic Medical Record (EMR). Experience with Allscripts or Epic is highly preferred
• Able to effectively communicate with patients to ensure follow-through with referrals via phone communications
