This role is essential in coordinating specialty care, managing documentation, and providing excellent customer service to patients and healthcare partners. If you are highly organized, detail-oriented, and passionate about patient care coordination, we encourage you to apply!Key ResponsibilitiesPatient Referrals & Pre-Authorizations:Obtain pre-authorizations in compliance with payer requirementsEnsure referral providers and facilities are in-network based on insurance coverageFacilitate referral scheduling, including direct coordination of appointments and assisting patients with potential barriersTrack and follow up on scheduled and unscheduled referrals to ensure patient complianceAcquire specialty notes following completed appointmentsCommunicate referral status to patients via phone or written correspondenceDiagnostic Order Coordination:Manage and track diagnostic orders throughout the order cycleSchedule appointments, obtain necessary prior authorizations, and provide timely patient notificationsFollow up on pending or incomplete orders, assisting patients in rescheduling as neededRefer patients with barriers (transportation, language, financial, etc.) to Case Management for additional supportCustomer Service & Communication:Provide excellent customer service to both internal teams and external healthcare partnersRespond to patient inquiries regarding referrals, pre-authorizations, and appointment processesEnsure proper documentation in the Electronic Health Record (EHR) system and maintain tracking logsQualifications & SkillsEducation: High School Diploma or EquivalentExperience:Two or more years of healthcare experience preferredStrong clinical and administrative skillsCulturally competent with experience working with diverse populationsTechnical Skills:Intermediate proficiency in Microsoft Office and EHR systemsHigh degree of accuracy, attention to detail, and multitasking abilityOther:Professional demeanor with strong communication skillsAbility to prioritize tasks and manage time efficientlyWhy Join Valley-Wide?Valley-Wide Health Systems, Inc. is dedicated to providing high-quality, patient-centered healthcare to all individuals across our multi-county service area in southern Colorado.
