Job summary

Valley-Wide Health Systems, Inc. is seeking a Referral Coordinator to support our clinical care teams by facilitating patient referrals, obtaining pre-authorizations, and ensuring a seamless referral process.

You will coordinate specialty care, manage documentation in the EHR, and provide excellent customer service to patients and healthcare partners, addressing barriers such as transportation or language when needed.

Qualifications

  • High School Diploma or equivalent required.
  • Two or more years of healthcare experience preferred.
  • Strong clinical and administrative skills.
  • Culturally competent with experience working with diverse populations.
  • Proficient in Microsoft Office and EHR systems.
  • Highly accurate, detail-oriented and able to multitask.

Responsibilities

  • Obtain pre-authorizations in compliance with payer requirements.
  • Ensure referral providers and facilities are in-network based on insurance coverage.
  • Coordinate referral scheduling and assist patients with barriers.
  • Track and follow up on referrals to ensure patient compliance.
  • Acquire specialty notes after appointments.
  • Manage diagnostic orders throughout the order cycle.
  • Schedule appointments, obtain necessary prior authorizations, and provide timely patient notifications.
  • Follow up on pending or incomplete orders, assisting patients in rescheduling as needed.
  • Refer patients with barriers to Case Management for additional support.
  • Respond to patient inquiries regarding referrals, pre-authorizations, and appointment processes.
  • Maintain documentation in the EHR and tracking logs.
