Job summary

The Old Colony YMCA in Hyannis, MA is seeking a Reentry Services Case Manager to join the CJSC team. The role conducts in-reach to Massachusetts correctional facilities, enrolling clients in the Ralph Gants Reentry Services Program and managing caseloads from intake to 18-month follow-up.

You will build rapport with clients, coordinate with housing, education, and treatment resources, and work with community mentoring programs to establish supportive connections.

Qualifications

• Strong oral, written, and presentation skills.
• Experience with case management in social services is favorable.
• Ability to engage high-risk populations with sensitivity.

Responsibilities

• Conduct in-reach to MA correctional facilities and jails to publicize CJSC services.
• Develop rapport with clients returning from incarceration to support warm handoffs.
• Create and maintain a case file with intake data, assessments, and reentry plans.
• Maintain monthly contact with clients for 18 months and document progress.
• Collaborate with CJSC Public Benefits Case Manager to secure benefits and referrals.
• Coordinate with community-based mentoring programs for client connections.
• Cross-train in risk/need assessment tools and contingency management.
• Attend meetings and participate in program operations and events.
