Job summary

Qualifications

• Experience as a behavioral health peer support worker.
• Ability to screen and assess members and plan services.
• Provide crisis intervention and case management support.
• Assist with documentation in electronic health records.

Responsibilities

• Transport members in a 10 passenger van to/from home, clinic, and field trips.
• Welcome members and manage sign-in/out at the center.
• Facilitate groups and 1:1 interactions, outreach calls, and chores.
• Complete group sheets and data entry per QA guidelines.
• Assist with intakes, outreach, and coordination with clinical team.
