The Recovery Outreach Case Manager serves as the primary outreach and engagement specialist for Mat‑Su Health Services’ Positive Pathways Program and related substance use disorder recovery initiatives. Under the supervision of the Rehabilitation Services Manager and in close collaboration with the SUD Integration Coordinator, the Recovery Outreach Case Manager provides trauma‑informed, recovery‑oriented case management services to individuals affected by substance use disorders and related social determinants of health. The role involves conducting community outreach, establishing rapport with individuals who may be hesitant to engage in treatment, and assisting participants in accessing medical, behavioral health, recovery support, housing, employment, and other community resources. Key duties include supporting participant engagement through recovery support groups, facilitating referrals and benefit applications, and promoting continued engagement in treatment and recovery services.
