Clarvida is seeking a dedicated Recovery Coordinator in Taft, CA to provide outreach, case management, and mental health rehabilitation services. The role includes counseling, crisis intervention, and coordinating assessments and follow-up care to empower clients toward stability and wellness.

Responsibilities include developing individualized treatment plans, maintaining regular contact (including home visits), and linking clients with community resources while adhering to HIPAA and regulatory

Formación

• AA/AS/BA in human services, psychology or related field plus 1 year related experience.
• Minimum 1 year of case management or psychiatric treatment experience.
• Valid California driver's license and active auto insurance.
• Strong organizational and prioritization skills; excellent communication.
• Knowledge of rehabilitation model preferred.
• Proficient with computers (MS Office).

Responsabilidades

• Conduct client intake and comprehensive assessments.
• Develop individualized treatment plans with clients and families.
• Coordinate and manage services for care plan implementation.
• Provide counseling, crisis intervention, and skill-building.
• Maintain client contact, including home visits.
• Link clients to community resources and monitor outcomes.
• Transport clients using personal vehicle as needed.
• Document services and billing per regulations (Short Doyle/Medi-Cal).
• Maintain HIPAA compliance and professional boundaries.
• Meet monthly productivity requirements (100 hours).
