Qualifications

• Requires High School Diploma or GED.
• Bachelor's degree in a human services field preferred.
• Minimum of one year in mental health or psychosocial rehabilitation preferred.
• Valid driver’s license with acceptable driving record.

Responsibilities

• Provide transportation to appointments, shopping, and other activities.
• Assist with money management and budgeting.
• Support scheduling and attending appointments.
• Provide diet coaching and encourage healthy lifestyles.
• Assist with shopping, cleaning, and household tasks.
• Support activities of daily living (ADLs) while promoting dignity and independence.
• Teach and reinforce independent living and life skills.
• Encourage personal responsibility and self-determination.
• Ensure curfew expectations are followed.
• Perform overnight checks 10:00 PM–6:00 AM as assigned.
• Monitor member safety and communicate concerns to supervisors.
• Maintain accurate documentation of interactions and activities.
• Maintain confidentiality and professional boundaries.
• Collaborate with team to support member goals and service plans.
• Use respectful, non-stigmatizing language.
