Responsibilities include motivational, stage-matched support, goal attainment across wellness domains, and coordination across care teams.

Qualifications

• Must have High School diploma or GED.
• Relevant degree in human services preferred (Assoc/Bachelor).

Responsibilities

• Delivering individualized, stage-matched support grounded in motivational and strength-based approaches.
• Support clients in developing and achieving personalized recovery goals across wellness dimensions.
• Coordinate services across mental health, physical health, and substance use treatment domains.
• Provide skill-building in daily living, symptom management, housing, education and employment.
• Connect clients to healthcare, benefits, and housing resources.
• Assist with crisis planning, discharge coordination, and collaboration with families and other systems.
• Facilitate or co-facilitate group services such as early recovery, WRAP, and family support.
• Collaborate with ITCD team to ensure integrated, high-quality care.
• Provide transportation support and accompany clients to appointments.

Education
