You will conduct individual and group counseling using evidence-based methods and collaborate with a multidisciplinary team on comprehensive treatment plans. The role involves monitoring progress, adjusting interventions, and developing discharge plans to ensure smooth transitions to the next level of care.

Qualifications

• Tier 1 Counselor requires High School Diploma / GED or Counselor Intern.
• Tier 2 Counselor requires CADC, LAC, or LSW.
• Tier 3 Counselor requires Master’s degree and licensure (LCADC/LCSW/LPC).

Responsibilities

• Conduct individual and group counseling sessions using evidence-based practices.
• Collaborate with a multidisciplinary team to develop treatment plans.
• Monitor and assess patient progress and adjust interventions.
• Develop discharge/continuum of care plans for appropriate levels of care.
• Maintain patient caseload and complete charting as required.
