Job summary

Qualifications

• High school diploma or equivalent required; peer support training or ability to obtain certification within one year of hire.
• Personal lived experience with mental health, substance use, co-occurring, or related life challenges.
• Experience working with diverse age groups and families; strong written and verbal communication; recovery-oriented approach.

Responsibilities

• Provide 1:1 peer support to individuals and families in homes and community settings, building trust and promoting recovery.
• Help participants navigate mental health, substance use, and other systems to ensure needs are addressed.
• Identify strengths, barriers, needs, goals, and priorities with participants and families.
• Coordinate with care teams to develop plans of care and crisis plans.
• Provide coaching, education, and practical support to strengthen wellness and self-advocacy skills.
• Link participants to services and community resources for sustainable support networks.
• Attend supervision and required meetings and document progress timely and accurately.
