Job summary

Elwyn is seeking a Recovery Coach in Philadelphia to support individuals after screening, helping enroll in services and develop a tailored action plan. You will work within an Integrated Team to address mental health recovery, daily living skills, relationships, and community employment while utilizing trauma-informed strategies.

You will collaborate with peers, provide counseling, and document progress according to policy. A valid driver’s license and lived recovery experience are required.

Qualifications

• High School Equivalency required
• AA or BA in Human Service field preferred
• Recovery Coach Certification required and/or completion within one year of hire
• Must possess lived experience and one year of addictions recovery
• Must have strong time management and organizational skills
• Excellent customer interaction, collaboration, presentation, and written and verbal communication
• Current, valid driver's license in state of residence and two years driving experience in the United States

Responsibilities

• Meet newly enrolled individuals after screening to assist with enrollment and begin developing therapeutic rapport
• Assist individuals with identifying meaningful goals for their Individual Action Plan/Treatment Plan
• Develop a comprehensive IAP/Treatment with the Integrated team ensuring a multidiscipline approach
• Practice a trauma-informed approach to treatment planning and service delivery
• Work with individuals through life transitions and develop Crisis Management Plan
• Promote and integrate Recovery Learning Community and peer-led activities
• Provide peer counseling, support, and education to validate life experiences
• Maintain clear and concise clinical documentation in compliance with policy and CARF standards
• Engage in non-judgmental listening and share personal recovery experiences to build peer relationships
• Facilitate understanding of individuals' perspective and rights with the team

Job Description

POSITION SUMMARY

The Recovery Coach meets with newly enrolled individuals after their screening review to assist with the enrollment process and begin developing therapeutic rapport to support their addiction recovery. As part of the Integrated Team, the role assists individuals in identifying meaningful goals for their Individual Action Plan/Treatment Plan, focusing on areas such as mental health recovery education, daily living skills, recovery from addictions, relationships, vocational activities, and leisure.

DUTIES AND RESPONSIBILITIES

• Meet newly enrolled individual's after screening is reviewed, to assist with enrollment process and begin to develop therapeutic rapport.


• As part of the Integrated Team, assist individual's with identifying meaningful goals for their Individual Action Plan/Treatment Plan focusing on the following areas: mental health recovery education, daily living skills, recovery from additions, relationships, vocational & leisure.


• In conjunction with the individual and Integrated team develop a comprehensive Individualized Action Plan (IAP)/Treatment, per Program Regulation. Ensures a multidiscipline approach is taken, using evidence-based practices which support recovery.


• Practice a trauma informed approach to treatment planning and service delivery.


• Work with individuals through life transitions and collaborates with individual and Integrated Team to develop the Crisis Management Plan.


• Integrate the PRISMModel for effective treatment of serious and persistent mental illness, into everyday practice and clinical intervention.


• Promote integrated services that meet the complex needs of individual's recovery goals


• Develop interventions that encompass all the individual's life domains including but not limited to the following:

• Facilitates peer lead activities, empowering enrolled individuals to take a lead in the planning and implementation process


• Develops, in conjunction with peer lead groups, information related to wellness and recovery


• Engages in non-judgmental, compassionate active listening and shares personal recovery experiences to build peer relationships


• Provides peer counseling, support and education to validate individual's' life experiences.


• Works with individuals to identify and utilize preferred recovery tools and self-care techniques (i.e. WRAP, peer groups).


• Utilizes motivational interviewing techniques.


• Encourages individuals use of and involvement in the Recovery Learning Community, where applicable.


• Supports participation in individual's self-help & advocacy organizations that promote recovery.


• Provides training and consultation to team to facilitate understanding individual's' perspective and protection of individuals rights.


• Peer encourages on-going engagement and involvement of family members in all phases of service delivery and evaluation based on the desires of the individual this may include ensuring family members understand:

  • (2) the service delivery model
  
  
  • (3) the roles and functions of the Individual's Team members/Intensive GLE staff and how to contact them
  
  
  • (4) how the family will be supported. As agreed upon with individual develops communication plans with family members.
  
  
  
  


• Maintains clinical documentation/records that is clear and concise, reflecting individual's preferences, needs and recovery and in compliance with FHR/ DMH policies and procedures as well as CARF standards

EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS

• High School Equivalency required, AA or BA in Human Service field preferred.


• Recovery Coach Certification required and/or completion within one year of hire.


• Must possess lived experience (defined as having suffered from an addiction) and one Year of addictions recovery is required. Crisis Intervention experience preferred.


• Demonstrated strong time management and organizational skills


• Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions


• Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication


• Must have current, valid driver's license in state of residence, two (2) years driving experience in the United States, and acceptable driving record
