Job summary

Freeman Health Partners Medical Services, PC is seeking an LPC-MHSP Outpatient Counselor to deliver clinical assessments, diagnoses, and therapeutic services in an outpatient setting within Tennessee. You will collaborate with physicians, behavioral health professionals, and other team members to ensure coordinated care and recovery goals.

Qualifications

• Master’s degree in counseling or related field.
• Current LPC-MHSP licensure in Tennessee in good standing.
• Minimum 1 year of behavioral health or counseling experience; co-occurring disorders experience preferred.

Responsibilities

• Conduct comprehensive initial and ongoing clinical assessments.
• Diagnose and treat mental, emotional, behavioral, substance use, and co-occurring disorders.
• Develop and update individualized treatment plans with measurable objectives.
• Provide individual and group counseling and psychotherapy.
• Utilize CBT, MI, relapse prevention and other evidence-based modalities.
• Perform risk assessments and develop safety plans when indicated.
• Coordinate care with the interdisciplinary team and refer to higher/lower levels of care as needed.
• Maintain accurate documentation in the EHR per policies and laws.
• Ensure confidentiality and adhere to HIPAA and professional standards.
