Job summary

Foundations Minnesota, a Foundations Recovery Network member and a subsidiary of Universal Health Services (UHS), seeks a compassionate Recovery Care Coordinator (CPRS) to support clients in SUD and behavioral health treatment at our Eden Prairie Outpatient Treatment Center. The role emphasizes recovery-focused care, peer support, and care coordination.

The position is salaried, full-time, with standard benefits, and a morning schedule Monday through Friday.

Qualifications

• Bachelor's degree in behavioral science or related field, or other qualifying credentials.
• 2+ years of treatment coordination experience with substance use disorders.
• CPRS Certification must be held.
• Knows Minnesota Statutes Chapters 245G, 260E, 245A, 626.557 and 626.5572; CARF/state/federal regulatory knowledge.

Responsibilities

• Establish recovery-focused relationships to enhance engagement in treatment.
• Provide peer-based encouragement, advocacy, and recovery support.
• Identify client strengths, recovery goals, and wellness strategies.
• Meet with clients to assess barriers to treatment participation.
• Conduct outreach to support attendance and continuity in treatment.
• Facilitate engagement in recovery meetings and peer services.
• Help clients build healthy social supports and community connections.
