Job summary

Freeman Recovery Center in Tennessee seeks a dedicated Case Manager to support individuals on their recovery journey from addiction and/or mental illness. You will perform assessments, build recovery plans, and coordinate care with a wide network of providers.

You will advocate for clients, provide emotional support, educate on coping strategies, and document services while collaborating with psychiatrists, nurses, social workers, and other team members to ensure integrated care.

Qualifications

• Previous experience in case management or mental health settings is preferred.
• Experience with recovery or crisis work is beneficial.
• Strong interpersonal and communication skills are essential.
• Good organizational, problem-solving, and teamwork abilities are important.

Responsibilities

• Conduct initial and ongoing assessments of clients’ mental health and substance use.
• Develop personalized recovery plans with clients.
• Coordinate with healthcare providers and community resources.
• Advocate for clients’ rights and preferences.
• Provide emotional support throughout recovery.
• Educate clients on mental health, treatment options, coping strategies and relapse prevention.
• Teach life skills like budgeting and time management.
• Document all interactions and outcomes accurately.
• Collaborate with psychiatrists, nurses, social workers, and other case managers.
