Job summary

NET Centers/NET Community Care is seeking a Certified Peer Support Specialist to assist adults with serious mental health challenges in Philadelphia County. Responsibilities

Works with participants to assess strengths and needs to develop a person-centered, individualized service plan.
Supports the participant in carrying out their recovery plan and reviews needs.
Works with participants to identify and strengthen supports for community integration.
Monitors supports for appropriateness and effectiveness.
Provides advocacy at all levels on behalf of individuals.
Ensures visits are documented in the EHR.
Achieves a minimum of 50 billable hours per two week pay period.
