This position ensures compliance with Missouri Department of Health and Senior Services (DHSS) guidelines, accurately documents findings, and provides person-centered, professional interaction with clients, caregivers, and providers.ESSENTIAL DUTIES AND RESPONSIBILITIESThe essential duties and responsibilities include the following. Other duties may be assigned.Conduct Medicaid Reassessments in clients’ homes in accordance with DHSS requirements and agency standards.Evaluate clients’ medical, functional, and social needs to determine continued eligibility and level of care.Document assessments clearly and accurately in the designated software systems within required timeframes.Verify care plan tasks and time utilization, ensuring all services align with client needs and policy.Communicate with providers, family members, and informal supports to gather collateral information as needed.Maintain current knowledge of program regulations, policies, and procedures.Participate in quality assurance reviews and corrective action processes.Adhere to confidentiality, professionalism, and ethical standards in all interactions.Perform other duties as assigned to support the goals of the Senior Services Department.SKILLS AND ABILITIESTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
