Job summary

The Bridge Project is seeking a dedicated Recovery Coordinator to join our Treatment Team on a full-time basis. The role focuses on care coordination, screening and recovery planning for service users within a multidisciplinary setting.

You will work as the main point of contact for a defined caseload, delivering asset mapping, assessments and ongoing reviews to support sustained recovery from alcohol and drug issues. A DBS check is required.

Qualifications

  • 2 years’ full-time experience of working with people with alcohol and/or drug problems and/or people with complex and multiple needs within the last 4 years.
  • Experience of case management and delivering care coordination in drug and alcohol or related field.
  • Highly organised with excellent communication skills and ability to work as part of a team.

Responsibilities

  • Act as the main care coordinator for an assigned group of service users.
  • Provide screening, asset mapping, comprehensive assessments, recovery planning and reviews of care.
  • Deliver high-quality drug and alcohol treatment provision within the MDT.
  • Support service users from point of entry through their treatment and recovery journey.

Job description

Are you a Recovery Coordinator looking for a new role, if so we would love to hear from you! We are looking for a full-time permanent Recovery Coordinator to work in our Treatment Team.

New Vision Bradford is a partnership between the Bridge Project, Create Strength Group, Project 6 and Waythrough. Under this umbrella, the partnership delivers an adult drug and alcohol service for the Bradford and Craven District, aiming to help people tackle the use of alcohol and drugs and reduce risk taking behaviours through dedicated prevention, intervention, and support.

This role will work as an integral part of the multi-disciplinary team delivering high-quality drug and alcohol treatment provision, by supporting service users from point of entry into the service and throughout their treatment and recovery journey. You will play a crucial role acting as the main care coordinator for an assigned group of service users, providing screening, asset mapping, comprehensive assessments, recovery planning and reviews of care.

To be considered for this role you will need 2 years’ full-time experience of working with people with alcohol and/or drug problems and/or people with complex and multiple needs within the last 4 years, as well as being able to demonstrate experience of case management and delivering care coordination in drug and alcohol or a directly related field.

You will be highly organised, with excellent communication skills and able to manage your time effectively, working as part of the team with a co-operative approach to supporting colleagues delivering the service objectives, to ensure a high-quality service is maintained, whilst applying your knowledge and understanding of drug and alcohol issues.

There is an element of flexibility around working hours required by the postholder. A

willingness to undergo an enhanced DBS check is also a requirement of the post (the results of which would not necessarily exclude applicants from consideration).
