Job summary

The Sunrise Recruitment Ltd seeks an experienced social worker to assess needs and risks for adults, children and families in social care. You will develop and review personalised care plans in partnership with service users and their families, safeguarding vulnerable individuals and fulfilling statutory duties.

You will collaborate with health, housing, education and voluntary sector partners, maintaining accurate records and advocating for clients.

Responsibilities

• Develop, implement and review personalised care plans with service users and families.
• Safeguard vulnerable individuals and respond to abuse or neglect concerns.
• Perform statutory social work duties per legislation, policies and standards.
• Build professional relationships with service users, carers, families and professionals.
• Coordinate with multi-agency partners, including health, housing and education services.
• Monitor and review effectiveness of care packages and interventions.
• Maintain confidential, up-to-date case records and professional reports.
• Advocate for service users’ rights, independence, wellbeing and participation in decisions.
• Manage caseload and ensure timely assessments, reviews and interventions.

Monitor and review the effectiveness of care packages, interventions and support services.

Maintain accurate, confidential and up-to-date case records, assessments and professional reports.

Advocate for service users and promote their rights, independence, wellbeing and participation in decision-making.

Manage an allocated caseload effectively, prioritising urgent needs and ensuring assessments, reviews and interventions are completed within required timescales.
