REACH-NC Case Manager – Duplin and Pender County

JOB TITLE: REACH-NC Case Manager – Duplin and Pender County

LOCATION: Duplin and Pender County (Burgaw, NC)
SCHEDULE: Full-Time, Non-Exempt (40 hours/week)

Coastal Horizons Center, Inc. is seeking a qualified Case Manager to join our Reentry and Engagement for Alternative to Crime through Habilitation in North Carolina (REACH-NC) program. This full-time position, serving Duplin and Pender counties, offers an excellent opportunity for someone passionate about helping justice-involved individuals access services and support that promote wellness and recovery.

Primary Duties Include:

  • Conduct psycho-social screenings and assessments for individuals referred to the REACH-NC program.
  • Develop individualized service plans in collaboration with participants and multidisciplinary teams.
  • Coordinate and link participants to behavioral health, physical health, educational, and vocational services.
  • Monitor progress and engagement; advocate for client access to needed services.
  • Maintain accurate and timely documentation, including data collection required by grant funding.
  • Build trusting relationships with clients to foster engagement in services.
  • Provide case management services with availability beyond traditional business hours as needed.

Required educational/experiential qualifications:

  • High school diploma or higher required.
  • Two (2) years of experience working in justice and/or behavioral health settings required.
  • Preference for individuals with certification from the NC Addiction Specialist Professional Practice Board and/or NC Peer Support Specialist certification.
  • Knowledge of recovery, harm reduction, and wellness models.
  • Lived experience with recovery and/or the justice system preferred.
  • Strong communication, organization, and collaboration skills.
  • Ability to work with diverse populations and community partners.
  • Proficiency in basic computer programs (Word, Excel, email).
  • Spanish/English bilingual skills are a plus.

Additional Requirements:

  • Valid North Carolina driver’s license and vehicle insurance.
  • Willingness to work occasional non-traditional hours based on participant needs.
  • A valid driver’s license and/or government-issued ID is required. Must have reliable transportation.
