Job summary

Cumberland County is seeking a full-time Re-entry Program Coordinator to oversee day-to-day operations, coordinate planning services, and support reintegration efforts to reduce recidivism. The position works under the Sheriff’s Office with a comprehensive benefits package.

Responsibilities include screening and risk assessment, coordinating referrals, leading weekly release planning meetings, maintaining client caseloads, and reporting program outcomes to funders.

Qualifications

  • Bachelor's or Master's degree in social work, criminal justice, public administration, or a related field.
  • LCSW preferred.
  • Experience with individuals in the criminal justice system, preferably in reentry or corrections.
  • Knowledge of reentry principles and evidence-based practices.
  • Strong organizational, planning and project management skills.

Responsibilities

  • Screen and triage inmates using validated instruments and conduct full risk/need assessments.
  • Coordinate referrals for reentry planning services based on risk and need.
  • Lead weekly release planning meetings with partners and manage information sharing.
  • Maintain an active caseload and provide reentry planning services for high-risk individuals.
  • Facilitate access to housing, job training, healthcare, counseling and legal assistance.
  • Provide ongoing support, advocacy, crisis intervention as needed; monitor progress.
  • Maintain records for grant reporting and program evaluation; produce reports for funders.

Job description

JOB PURPOSE: Coordinates the day to day activities of the re-entry program and provides re-entry planning services. Selected candidate will work collaboratively with various stakeholders to ensure seamless coordination of services and resources to support successful reintegration and reduce recidivism rates. Work is performed under supervision and support of the Sheriff’s Office. This position is fulltime with a full benefits package. Pay for this position is $27.62 per hour This amount reflects the County’s good-faith estimate of pay for this position at the time of posting. Starting pay is determined based on qualifications and experience in alignment with the collective bargaining agreement.

QUALIFICATIONS:
  • Bachelor's or Master's degree in social work, criminal justice, public administration, or a related field.
  • Licensed Clinical Social Worker (LCSW) preferred.
  • Previous experience working with individuals involved in the criminal justice system, preferably in a reentry or corrections setting.
  • Knowledge of reentry principles, evidence-based practices, and best practices in criminal justice and social services.
  • Strong organizational, planning, and project management skills.
  • Excellent interpersonal and communication skills, with the ability to collaborate effectively with diverse stakeholders.
  • Ability to work independently, prioritize tasks, and manage multiple responsibilities in a fast-paced environment.
  • Commitment to promoting equity, diversity, and inclusion in service delivery and program development.
KEY EXPECTED RESULTS:
  1. Program Development: Design, implement, and evaluate reentry programs and initiatives aimed at addressing the needs of individuals returning from jail to the community. Develop strategies to promote successful reintegration, enhance public safety, and reduce the risk of reoffending.
  2. Stakeholder Collaboration: Build and maintain effective partnerships with jail staff, probation/parole officers, law enforcement agencies, social service providers, community organizations, and other relevant stakeholders. Coordinate efforts to streamline communication, facilitate information sharing, and enhance collaboration in supporting reentry efforts.
  3. Screening and Assessment – Screen all new inmates using validated instrument (e.g., AC-OK) and triage for and conduct full risk and need assessment using validated instruments (e.g., LSI-R, ASAM).
  4. Coordinate referrals – Receive, review, triage, assign and track referrals for reentry planning services based on level of risk and need.
  5. Coordinate collaborative release planning: Convene and lead weekly release planning meetings with jail and community-based partners, including but not limited to reentry case managers, MAT nurse, peers and recovery coaches. Secure and manage releases of information.
  6. Provide reentry planning services – Maintain an active caseload and provide reentry planning services to individuals at high risk of recidivism and overdose in Cumberland County Jail. Develop and maintain awareness of and relationships with relevant community-based services and supports to support a comprehensive spectrum of reentry needs, warm handoffs, and safe transitions into the community.
  7. Resource Coordination: Facilitate access to a wide range of services and resources to support successful reentry, including housing assistance, job training, vocational rehabilitation, healthcare services, counseling, peer support, and legal assistance. Connect clients with community-based programs and support networks to promote stability and self-sufficiency.
  8. Monitoring and Support: Provide ongoing support, guidance, and advocacy to individuals throughout the reentry process. Monitor progress, address challenges and barriers, and provide crisis intervention as needed. Offer encouragement, motivation, and empowerment to help clients achieve their reintegration goals and maintain positive momentum.
  9. Data Management and Reporting: Maintain accurate and up-to-date records for grant reporting and program monitoring including client interactions, program activities, outcomes, and performance metrics. Present residents with informed consent forms, as needed, and safely store and track informed consent for program evaluation projects. Prepare regular reports and documentation for funders, stakeholders, regulatory agencies, and organizational purposes. Use data to assess program effectiveness, identify areas for improvement, and inform decision-making.
