Job summary

Elwyn is hiring a Re-Entry Case Manager to support individuals housed in the Wake County Detention Center as they prepare for release. You will coordinate warm handoffs to community partners and link clients to essential resources and services upon release, working with a multidisciplinary team under clinical supervision.

The role requires trauma-informed, person-centered care, and a housing-first approach.

Qualifications

• Bachelor's in behavioral science preferred, or 5+ years of field experience considered
• 3+ related experience working with individuals with mental health conditions, preferred
• Must complete ACCSModule Training within 30 days of hire
• Participate in other ELWYN required/recommended training according to the training calendar
• Meet or exceed professional development goals
• Ability to make contacts and establish relationships with internal and external supports

Responsibilities

• Integrates the Prism Model for effective treatment of serious and persistent mental illness into everyday practice and intervention.
• Provides person-centered, strength-based, trauma-informed care.
• Ensures clients receive effective, understandable, and respectful care that is culturally appropriate and in preferred language.
• Reviews outcomes to evaluate community tenure and independence with the team.
• Provides case management for an assigned group of clients, coordinating and monitoring activities.
• Practices a Housing First approach and uses evidence-based assessment tools.
• Develops the Community Service Plan with the client and team and updates as needs change.
• Provides de-escalation techniques to manage crises and ensure safety.
• Documents client progress and outreach in the Electronic Health Record.
• Engages clients across the adult lifespan and supports daily living activities.

Responsibilities

The Re-Entry Case Manager works with individuals housed in the Wake County Detention Center who are preparing for release from jail. This role facilitates and coordinates warm handoffs to community partners and connects individuals to essential resources and services. The Re-Entry Case Manager divides their time between the detention center and the community, providing transportation when needed and ensuring that individuals are successfully linked to supports and services upon release. As a member of a multidisciplinary team, the Re-Entry Case Manager uses their skills and experience to assess, plan, develop, coordinate, and provide treatment, rehabilitation, and support services to jail clients under the clinical supervision of the Forensic Director. ESSENTIAL DUTIES AND RESPONSIBILITIES

• Integrates the Prism Model for effective treatment of serious and persistent mental illness, into everyday practice and intervention.
• Provides person-centered, strength-based, trauma-informed care.
• Ensures that clients receive effective, understandable, and respectful care that is provided in a manner compatible with client's cultural beliefs and practices and preferred language.
• Along with the team, on an ongoing basis, reviews the required outcomes to evaluate community tenure and independence.
• Provides service coordination (case management) for an assigned group of clients, including coordination and monitoring of activities, assumes responsibility for, implementing, evaluating, and revising the overall Community Service Plan in collaboration with the client and team; symptom management, works with the clinical team, ensuring immediate changes are made in the Community Service Plan as clients' needs change; educate and support clients' families, and advocate for clients rights and preferences.
• Practices a Housing First approach
• Utilizes evidence-based assessment tools that support matching a client's needs to their services
• In partnership with the client and team develops the Community Service Plan.
• Provides de-escalation techniques to aid the client (s) in managing a crisis, providing intervention to decrease risk, manage symptoms, and maintain safety
• Participates in the Daily Organizational Meeting
• Documents client progress, activities, and outreaches in the Electronic Health Record
• Practices engagement skills across the adult lifespan
• Support: provides practical help and support, advocacy, coordination, problem-solving, training, and supervision to help clients obtain the necessities of daily living; assist clients in finding and maintaining safe housing, coaches clients to learn and carry out ADLs, assist and support clients to perform household activities, including house cleaning and laundry, teach money-management skills, nutrition education, meal planning, grocery shopping, access transportation, help clients to have and efficiently use primary care, dentist, and other medical specialist, support clients with structuring their time (community resources, social and leisure activities, hobbies, etc.)
• Awareness of and offers a range of valuable community activities and linkages that support the client's Community Service Plan.
• Performs other duties as assigned
