Trillium Health Resources, a Tailored Plan and MCO serving 46 North Carolina counties, seeks a Re-Entry Care Coordinator to assist justice-involved individuals transitioning between care settings. The role focuses on care coordination, transition planning, and cross-provider collaboration to support stable outcomes.

Qualified professionals, including RN or Master’s degree in human services with relevant experience, are encouraged to apply.

Qualifications

  • Must meet Qualified Professional (QP) status per NC requirements, with at least one year of experience with the forensic population.
  • Fully licensed RN who is licensed in NC with four years of MH/DD/SA experience.
  • Master’s degree holder with one year of MH/DD/SA experience, or a substance abuse professional with one year of supervised experience.
  • Bachelor’s in human services with two years MH/DD/SA experience, or related supervised experience.
  • Bachelor’s in a non-human-services field with four years MH/DD/SA experience.

Responsibilities

  • Coordinate and manage care transition activities for recipients on the High Priority Re-entry list.
  • Develop and implement transition plans based on individual needs.
  • Collaborate with providers and partners to align services during transition periods.
  • Monitor transitions and intervene to resolve delays or barriers.
  • Facilitate service connections and ensure continuity following transitions.

Education

Office Location: Remote in North Carolina and within 75 miles of Onslow County.

What We’re Looking For

Trillium Health Resources has a career opening for a Re-Entry Care Coordinator to join our team! The Re-Entry Care Coordinator supports individuals transitioning between care settings who are justice-involved and preparing for release from Department of Corrections or county detention facilities by providing care coordination and transition planning services. The role develops transition plans, aligns services across providers and community partners, and addresses barriers to support successful and stable transitions.

On a typical day, you might:
  • Coordinate and manage care transition activities for recipients on the High Priority Re-entry list to support successful discharges and service changes.
  • Develop and implement transition plans based on individual needs and transition requirements.
  • Collaborate with providers and partners to align services during transition periods.
  • Monitor transitions and intervene to resolve delays or barriers.
  • Facilitate service connections and ensure continuity following transitions.

Qualifications

Required:

  • Must meet Qualified Professional (QP) status per NC requirements through one of the following, with at least one (1) year of experience with the forensic population (working with individuals involved in the criminal justice system).
    • Fully licensed by the NC governing board regulating Human Services professions; OR
  • Fully licensed RN who is licensed to practice in the state of NC by the NC Board of Nursing who also has four (4) years of full-time experience with the MH/DD/SA population served. OR
  • A graduate of a college or university with a Master’s degree in a human service field and has one year of full-time, pre or post-graduate degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has one year of full-time, pre or postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  • A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has two years of full-time, pre or post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; OR
  • A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre or post-bachelor's degree accumulated MH/DD/SA experience with the MH/DD/SA population, or a substance abuse professional who has four years of full-time, pre or post bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling.
  • Must have a valid driver’s license.
  • Must reside in North Carolina and within 75 miles of Onslow County.
  • Must be able to travel within catchment as required.
Preferred:
  • Degree in Human Services or Criminal Justice.
  • Education, training, and work experience that shows a history of learning and applying the skills needed to work with a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds.
