What We're Looking For

We are seeking an RCT Regional Care Manager to join our Care Management Team. This role is vital in ensuring members receive comprehensive, person-centered care tailored to their physical, mental, and social needs.

The Care Manager acts as the primary contact for members, collaborating with a multidisciplinary team including primary care providers, specialists, and guardians, to ensure continuity and quality of care.

On a Typical Day, You Might
  • Provide oversight, outreach, engagement, and support for members receiving Tailored Care Management.
  • Conduct comprehensive assessments to evaluate members' needs and the effectiveness of care plans.
  • Develop and monitor integrated care plans addressing physical health, mental health, social support, lifestyle, and community involvement.
  • Hold regular meetings with members, both in person and via phone.
  • Collaborate with providers to share relevant care information, including assessments and reassessments.

Qualifications

Education/Experience:

  • Must meet Qualified Professional (QP) status.
  • Fully licensed by the NC governing board for Human Services professions or as an RN licensed in NC with four years of experience with the MH/DD/SA population.
  • Or, a master's degree in a human service field with one year of relevant experience; or a bachelor's degree with two years of relevant experience; or a bachelor's degree in another field with four years of relevant experience in the MH/DD/SA population.
