Role Overview

As an RCT Regional Care Manager, you will ensure members receive comprehensive, person-centered care tailored to their physical, mental, and social needs. You will serve as the primary contact for members and collaborate with a multidisciplinary team to promote continuity and quality of care.

Key Responsibilities
  1. Provide oversight, outreach, engagement, and support for members receiving Tailored Care Management.
  2. Conduct comprehensive assessments and evaluate care effectiveness.
  3. Develop and monitor integrated care plans addressing health, mental health, social support, and lifestyle factors.
  4. Hold regular meetings with members, both face-to-face and via phone.
  5. Coordinate with providers to share relevant care information.
