What We’re Looking For

We are seeking an RCT Regional Care Manager to join our Care Management Team. This role is crucial in providing comprehensive, person-centered care tailored to meet complex physical, mental health, and social needs of our members.

The Care Manager acts as a primary contact for members, collaborating with a multidisciplinary team—including primary care providers, specialists, care extenders, and legal guardians—to ensure continuity and quality of care.

On a Typical Day, You Might
  1. Provide oversight, outreach, engagement, and support for members receiving Tailored Care Management.
  2. Conduct comprehensive assessments to evaluate and address members’ needs.
  3. Develop and maintain integrated care plans covering physical health, mental health, social support, lifestyle, community involvement, and risk management.
  4. Hold regular meetings with members, both face-to-face and via telephone.
  5. Coordinate with providers to share relevant information, including assessments and care plans.
