Posting Salary Range: $102,801 - $113,621

What We’re Looking For

Trillium Health Resources has a career opening for an RCT Director of Clinical Care Management to join our team. The RCT Director of Clinical Care Management provides clinical leadership and supervision to our Regional Care Managers and their Care Teams and is responsible for developing and implementing policies and procedures to enhance cost effectiveness, while providing quality care to the populations served. Primary responsibilities include ensuring Waiver Contract requirements for Tailored Care Management are met and sustained, and maintaining a positive collaborative relationship with hospitals, network providers, stakeholders, and our members.

On a typical day, you might:

  • Provide direct supervision to RCT Regional Managers.
  • Implement and monitor processes to meet contractual requirements for Tailored Care Management.
  • Review and analyze reports and dashboards and use the information for developing improvement strategies.
  • Monitor case mix and caseload sizes of care teams and adjust resources as needed to ensure effective delivery of services.
  • Collaborate with Regional Managers to ensure care plans are properly written and identify the appropriate services.
  • Evaluate the appropriateness of referrals to the Regional Care Teams.
  • Participate in Clinical Operational meetings.

Qualifications

Required:

  • Master’s degree in Psychology, Social Work, Counseling, Human Services field, Psychiatric, or Nursing field and a minimum of three (3) years of leadership experience in care management or care coordination within the public health sector or Managed Care.
  • Fully licensed by the State of North Carolina as one of the following: PhD, Licensed Clinical Social Worker (LCSW), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Addiction Specialist (LCAS), or Licensed Psychological Associate (LPA).
  • Must have a valid driver’s license
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.

Preferred:

  • Supervision of two (2) or more employees.
  • Experience as a Manager or to four (4) or more employees preferred.
