Job summary

Cascadia of Lewiston is seeking a highly motivated nurse for a hybrid role that rotates between Nurse Manager responsibilities and floor nursing duties in a fast-paced post-acute rehab setting with 34 beds. The Nurse Manager portion includes wound care oversight, infection prevention, admissions support, and clinical leadership.

Floor shifts are 8 hours with medication administration, patient assessments, and direct care.

Qualifications

  • Degree in Nursing from an accredited nursing school.
  • Current LPN or RN license
  • Valid CPR certification.
  • Six months experience in a long-term care environment preferred.
  • Minimum one year supervisory experience in a hospital, nursing facility, or related health care facility.

Responsibilities

  • Oversee wound care, infection prevention, admissions support, and clinical leadership duties.
  • Perform floor nurse duties: medication administration, assessments, and direct patient care.
  • Use EMR to document and validate patient parameters; communicate changes via SBAR.
  • Collaborate with care team; ensure complete medical records and patient education.
  • Ensure punctuality and attendance for assigned shifts.

Job description

Position Summary

We are seeking a highly motivated nurse for a hybrid role that rotates between Nurse Manager responsibilities and floor nursing duties. Our facility is a fast-paced, post-acute, rehabilitation-focused setting that provides short-term care only, with a 34-bed capacity. In the Nurse Manager role, responsibilities will include wound care oversight, infection prevention, admissions support, and related clinical leadership duties. Floor nursing shifts are 8 hours and include medication administration, assessments, and direct patient care. We are looking for a positive, focused team player who is committed to providing high-quality care and supporting a collaborative work environment.

Essential Functions
  • Evaluates updates resident/patient health care plans to achieve person-centered care.
  • Conducts assessments for new residents/patients.
  • Actively participates in clinical meetings.
  • Collaborates with other members of the health care team, as needed.
  • Utilizes the electronic medical record to establish & validate parameters are established as indicated.
  • Establishes, monitors, and documents person-centered care, as indicated.
  • Uses SBAR to communicate patient's change of condition with physician.
  • Reviews test results from medical exams.
  • Implements physician orders and follow up with treatment plans.
  • Communicates with families, health providers and patients. Receives consent, as required.
  • Documents in the medical record to clearly reflect patient care & current medical condition.
  • Ensures completeness of medical records.
  • Provides emotional support to families and patients.
  • Provides education about health care plans to patients and families.
  • Collaborates with other clinical staff to successfully implement patient plans of care & provides excellent customer service.
  • Ensures punctuality and regular attendance for assigned shifts.
Other Functions
  • Obtains accurate information from physicians, residents/patients, and payor source(s) regarding the expected discharge plan and communicate this information to the interdisciplinary team at the facility.
  • Communicates information to care team and coordinate patient's smooth transition to the next level of care.
  • Coordinates referrals from hospitals' social service and discharge planning departments, physicians, case managers, insurance companies and other referral sources.
  • Conducts job responsibilities in according with the standards set out in the company's Code of Conduct, its policies and procedures, applicable federal and state laws, and applicable professional standards.
  • Performs other duties as assigned.
Knowledge/Skills/Abilities
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Strong communication, excellent customer service, teamwork and organizational skills.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
Requirements
Education

Degree in Nursing from an accredited nursing school.

Licenses/Certification

Current LPN license or Current RN License

Valid CPR certificate.

Experience

Six months experience in a long-term care environment preferred.

Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
