Job summary

Imagine the Possibilities, Inc. is seeking a Direct Support Professional to provide person-centered care in a Residential Care Facility setting.

You will work with the care team to help individuals achieve personal goals, implement behavioral plans, and ensure safety and dignity during daily activities. You will support medication administration, health monitoring, and documentation while following program plans and rights restrictions as directed.

Qualifications

Responsibilities

• Collaborate with the RCF Administrator and Program Supervisor on individual goals and implement program plans.
• Support individuals with behavioral plans using approved strategies while promoting independence and rights.
• Maintain a safe, respectful environment and complete required data and incident reporting.
• Safely administer medications, monitor vital signs, and manage medical supplies as applicable.
• Attend to scheduling needs, ensure safe food handling, and support daily operations.

This role operates in a Residential Care Facility (RCF) Setting, which is a type of facility that provides services for people with mental illness and other disabilities (Iowa Health & Human Services). Rather than institutionalizing individuals who have these physical and mental health needs, RCF settings provide these individuals with the opportunity to live and receive services in a way honoring to them and their abilities.

• Individual Goals & Person-Centered Support: Collaborate with the RCF Administrator and/or Program Supervisor on individual goals and implement program plans as written, including active treatment, range-of-motion exercises, ADLs, meaningful activities, and community participation.
• Behavioral Support, Choice & Advocacy: Use approved behavioral strategies and programs to support individuals while promoting independence, personal choice, dignity, and decision-making; apply rights restrictions only as directed by the IDT and individual service plan.
• Direct Support & Safety: Maintain a safe, respectful environment using the least restrictive approach; complete required data and incident reporting, support personal appearance and cleanliness, respect individual property, and contribute to a positive mealtime and household atmosphere.
• Medication & Health Support: Safely administer medications, treatments, and G-tube feedings as applicable; monitor PRN medications, vital signs, and side effects; complete required documentation and assessments; manage medication and medical supply inventory; and maintain a clean, organized medication area.
• Daily Operations & Food Safety: Maintain reliable attendance and follow scheduling and call-in procedures, including weekends and holidays; complete assigned shift duties and ensure safe food preparation, storage, sanitation, and handling practices.

Physical Demands

• TB testing in compliance with CDC guidelines and State recommendations.
• May require lifting at times, up to 35 pounds. Lifting more than 35 pounds requires the use of a two person lift, mechanical lift, or assistive equipment.
• May require prolonged sitting and working at a computer for up to eight hours.
• May require frequent bending, stooping, and stretching.
• May require prolonged standing for up to eight hours.
• Must wear hearing and eye protection in required areas.
• Must have ability to see objects/persons at a distance.
• Must have ability to see close work such as typed or handwritten material.
• Must have ability to hear conversations in a quiet and a noisy environment.
• Must have ability to determine where a sound is coming from.
• Must have ability to hear differences among bells, buzzers, beeps, horns, etc.
