Rapid Rehousing & Homeless Prevention Case Manager (SSVF)- Helena, MT

Join Veterans Inc., the largest provider of services to veterans and their families throughout New England, as a Rapid Rehousing & Homeless Prevention Case Manager (SSVF) - Helena, MT. We are seeking a motivated, goal-oriented professional with experience in human services to support Montana-based veterans.

Responsibilities:
  1. Conduct initial intake and assessments for veterans entering our programs.
  2. Develop individualized service plans focused on homelessness prevention and rapid rehousing.
  3. Assist veterans in accessing services such as VA benefits, employment, education, counseling, housing, and other public benefits.
  4. Track case management progress, maintain documentation, and report data monthly.
  5. Conduct outreach to promote programs and establish new referral sources.
Qualifications:
  • Bachelor's degree in Human Services, Social Work, or related field.
  • At least two years of experience in human services or working with veterans.
  • Ability to pass a CORI background check.
