Job summary

Vail Health seeks a Nurse Navigator to guide the patient care lifecycle within the department. The Navigator acts as the liaison between patients, families, and care providers across settings, ensuring seamless information flow and coordinated care.

The role emphasizes building patient-family relationships, developing care plans, and facilitating referrals and education. A Colorado RN license and BLS are required; an Associate Degree is needed for hires after mid-2018.

Qualifications

• RN license in Colorado or compact state
• Basic Life Support (BLS) certification required
• Associate Nursing Degree for positions hired after 07/01/2018

Responsibilities

• Consult with patients by phone or in person to determine needs and referrals
• Build a patient-family centered relationship
• Provide initial admission assessment and screenings
• Develop and deliver a Care Plan with treatment summary
• Organize evidence-based care plan and present to patients for discussion
• Follow up with patient and providers as needed
