Responsibilities

• Obtain and document health history with focused exam and care plan.
• Collaborate across teams to manage primary, acute and complex care.
• Prescribe and adjust therapies per scope of practice and guidelines.
• Initiate referrals and coordinate transitions across care settings.
• Deliver specialty consultation within Radiation Oncology as needed.
