Job summary

Mount Sinai Medical Center seeks a coordinator for the Wellness & Preventive Management Program to support HEDIS and annual wellness activities. You will review medical records to abstract data, ensure data quality, and collaborate with leadership to close care gaps.

Responsibilities include data collection, monitoring performance, and coordinating with PCPs and clinics to meet quality measures. Requires high school diploma; BS preferred; health care experience preferred.

  • High School Diploma required.
  • Preferred: Bachelor of Sciences degree or related field, foreign medical graduate.

Responsibilities

  • Support the Wellness & Preventive Management Program (WPM) with HEDIS and Annual Wellness Visits.
  • Abstract clinical data elements from medical records for HEDIS reporting (e.g., lab results, BP readings).
  • Collect medical record data and identify improvements impacting HEDIS measures.
  • Review noncompliant reports and flag charts with missing measures.
  • Request quality data documentation from outside resources.
  • Collaborate with leadership to execute quality strategies.
  • Manage reports and monitor performance at group and provider levels.
  • Engage PCP and clinic staff to close quality gaps.
  • Upload HEDIS data to medical records and send to the health plan.
  • Perform other related duties as assigned.
