Job summary

AdvantageCare Physicians (ACPNY) is seeking a Quality Navigator (RN) to embed within practices and drive Medicare Stars, HEDIS, QARR, and value-based care performance improvements. You will partner with providers, office leadership and care teams to close care gaps, enhance chronic disease management, and promote patient engagement across New York City metro clinics.

Qualified candidates hold a NYS RN license, a BSN, and at least two years of clinical experience, including ambulatory settings.

Qualifications

• Minimum of 2 years of clinical experience.
• Experience in Medicare Advantage, Managed Care, Population Health, Quality Improvement, Care Management, or Value-Based Care.
• Knowledge of chronic disease management and care coordination principles.

Responsibilities

• Improve Medicare Stars, HEDIS, QARR, and value-based care outcomes.
• Identify and close care gaps with providers and care teams.
• Conduct data-driven outreach to drive preventive and chronic care adherence.
• Document in Epic and track outreach outcomes.
• Support transitions of care and reduce readmissions.
