AdvantageCare Physicians is hiring a Quality Navigator (RN) to serve as an embedded clinical quality resource in NYC practices, focusing on improving Medicare Stars, HEDIS, QARR, and value-based outcomes.

The RN will partner with providers and care teams to close care gaps, optimize chronic disease management, and drive patient engagement. Requires BSN and NYS RN licensure; ambulatory care experience preferred.

Qualifications

• BSN required and NY RN licensure.
• Minimum 2 years clinical experience; ambulatory setting preferred.
• Experience with Medicare Advantage, Population Health, or Quality Improvement.
• Experience supporting HEDIS, QARR, Stars, NCQA programs.
• Knowledge of chronic disease management and care coordination.
• Experience with EMR and population health platforms.
• Strong analytics, communication, and patient engagement skills.

Responsabilités

• Improve Medicare Stars, HEDIS, QARR, and value-based care outcomes.
• Collaborate with providers, leadership, care management to close care gaps.
• Identify and pursue interventions to optimize chronic disease management.
• Perform data-driven outreach to drive preventive and chronic care adherence.
• Document in Epic and track outreach outcomes.
• Enhance patient experience and CAHPS performance.
• Support transitions of care and community health connections.
