HealthCare Partners, based in Garden City, NY, seeks a Quality Interventions Coordinator (Pharmacy). The role focuses on optimizing medication use, improving quality performance across CMS/Medicare, Medicaid and Commercial lines, and driving provider and member engagement.

You will lead outreach campaigns, educate teams on guidelines, and analyze performance data to close gaps in care. Field visits to offices and pharmacies may be required to advance measure improvement.

Qualifications

• Pharmacist with PharmD degree and active licensure.
• CRC certification required and must be maintained within 120 days of hire.
• Three+ years of experience in medication management and quality initiatives.

Responsabilités

• Drive performance on pharmacy-related quality measures (HEDIS, Star, QARR).
• Conduct high-volume telephonic outreach to providers, members, and pharmacies to close gaps in care.
• Educate providers and staff on clinical guidelines, coding, and documentation to support gap closure.
• Perform root cause analysis for coding, documentation, and data inaccuracies; create provider-level work plans.
• Conduct field visits to provider offices and pharmacies as needed to build relationships and support gap closure.
• Collaborate with providers, pharmacies, and members to update regimens and improve therapy outcomes.
