Job summary

Qualifications

• Active Indiana RN license in good standing.
• Home health experience preferred.
• Knowledge of Medicare Conditions of Participation and OASIS preferred.
• Strong organizational, analytical and communication skills.

Responsibilities

• Lead and coordinate the agency's QA/QAPI Program.
• Facilitate QAPI Committee meetings and maintain related documentation.
• Conduct clinical record audits for accuracy, completeness, reimbursement integrity, and regulatory compliance.
• Review OASIS assessments, plans of care, physician orders, and clinical documentation.
• Monitor quality measures, patient outcomes, hospitalizations, infections, incidents, and other key performance indicators.
• Analyze and trend quality data to identify opportunities for improvement.
• Develop, implement, and monitor performance improvement projects and corrective action plans.
• Provide education, coaching, and support to clinical staff regarding documentation requirements and best practices.
• Assist with regulatory survey preparedness and compliance activities.
• Collaborate with agency leadership to maintain a culture of quality, accountability, and continuous improvement.

Position Overview

The QA/QAPI Nurse serves as the agency's quality and compliance leader, overseeing quality assurance activities, clinical record review, performance improvement initiatives, and regulatory compliance efforts. This individual leads the QAPI Committee, analyzes agency quality metrics, identifies opportunities for improvement, and works closely with clinicians and leadership to promote best practices and positive patient outcomes. this is a Full-Time, Monday through Friday position.

This position is primarily office-based at our Evansville office. Occasional travel to patient homes, agency offices, meeting locations, educational events, and other assigned locations may be required to support quality improvement, compliance, operational, and educational activities.

• Active Indiana Registered Nurse (RN) license in good standing.
• Home health experience preferred.
• Knowledge of Medicare Conditions of Participation and OASIS requirements preferred.
• Strong organizational, analytical, and critical-thinking skills.
• Ability to work independently while collaborating effectively with interdisciplinary team members.
• Excellent written and verbal communication skills.
• Experience in quality assurance, compliance, utilization review, or performance improvement is a plus.
