Overview

Qualified Professional – Creedmoor IDD Unit (Regular). We are seeking an organized, positive, and compassionate Human Services professional to coordinate and monitor the array of services and supports needed to address the goals and desired outcomes of the people we serve. The Qualified Professional (QP) serves as the primary individual contributor, overseeing interdisciplinary care, developing person‑centered plans, and monitoring progress toward clinical and functional outcomes. The QP also supervises and integrates the work of Group Home Managers, Direct Care Associates, and other staff.

Salary: $46,200

Responsibilities
  • Assist each person we support to identify and communicate their requests and needs through direct interviews.
  • Supervise, integrate, and coordinate person‑centered plans and monitor progress toward personal, clinical, and functional outcomes.
  • Initiate periodic reviews, investigations, modifications, and adjustments by soliciting feedback as needs and goals change.
  • Review assessments completed by clients and other team members to help clients reach their full potential.
  • Write and supervise the implementation of person‑centered plans based on assessments, interviews, and observations.
  • Oversee residential activities, including home appearance, leisure materials, cleanliness, and community integration to ensure safety and health.
  • Visit vocational centers, residential homes, and other service locations to assess plan implementation.
  • Report to the Administrator and collaborate with colleagues to maintain communication, provide feedback, standardize procedures, expedite plan implementation, and improve employee performance.
  • Monitor activities to ensure quality assurance and state survey improvements.
  • Assist in recruiting, hiring, training, and dismissing Group Home Managers.
  • Serve as the link between clients and their families or legal guardians to gain approval for services provided.
Requirements
  • Bachelor’s degree in Social Work, Social Services, Human Services, or Special Education required; Master’s degree a plus.
  • Minimum 2 years experience working with persons with intellectual and/or developmental disabilities required; 1-year experience if paired with a Master’s degree.
  • Working knowledge of applicable regulations.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Ability to pass a drug screen and background check.
  • Physical ability to lift 20–50 pounds, pull 10 pounds, squat, kneel, crawl, crouch, climb, and stoop; proficiency in CPR from the floor level required.
  • Supervisory/management experience and experience writing individualized service plans preferred.
  • Excellent written and verbal communication skills.
  • Detail‑oriented with strong organizational skills.
Physical Requirements

Regularly required to lift 10 lbs; must be able to lift a minimum of 15 lbs and pull a minimum of 10 lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must regularly use hands to finger, handle, or feel objects, tools, or controls. Must be able to stand and walk regularly. Vision requirements include close, distance, and peripheral vision. Must be able to talk and hear.
