Job summary

Woodlake Legacy Senior Living in Lakewood, CO is seeking a qualified QMAP to support resident care and safe medication administration.

The role involves coordination with the Clinical Wellness Manager, medication documentation, ordering meds, and responding to emergencies. Candidates should have QMAP or MedTech certification and experience with seniors; a High School Diploma is required.

Responsibilities

• Administer medication to residents according to regulations.
• Complete medication documentation as outlined in policies and procedures.
• Order medications and supplies as needed.
• Report problems to Clinical Wellness Manager.
• Record incidents and changes in progress notes.
• Observe and report changes in residents' condition; document changes.
• Take resident vital signs as directed.
• Assist residents with ADLs as per care plans.
• Respond to emergencies and follow procedures.
• Plan duties with minimal supervision.

Now Hiring: QMAP

Responsibilities:

• Administer medication to identified residents according to State Medical Administration regulations.
• Complete medication administration documentation as outlined in policies and procedures.
• Order medications and supplies as needed.
• Report problems or errors to Clinical Wellness Manager.
• Accurately record and report incidents, errors, or accidents in appropriate documents on the eMAR system.
• Observe and report changes in the resident’s physical condition and cognitive/emotional status Clinical Wellness Manager as needed. Document changes in progress notes as outlined in procedures.
• Take resident vital signs as directed by Clinical Wellness Manager.
• Assist residents, as necessary, with Activities of Daily Living (ADL) as outlined in their Board and Care Plan (bathing, dressing, etc.)
• Respond to and act appropriately in emergency or disaster situations.
• Plan and complete duties with minimal direction from the supervisor.

Qualifications:

• High School Diploma or equivalent.
• QMAP or MedTech certification is required.
• Experience working with seniors.
