Job summary

Grace Pointe in Greeley, CO (1919 68th Avenue) seeks PRN QMAPs to join the Assisted Living/Memory Care team. You will help meet resident needs and administer medications as ordered by the attending physician, under supervision of the Charge Nurse and facility policies.

Qualified candidates must hold a QMAP certification. The salary range is $17.00-$19.00 per hour, based on experience, with shift differentials for nights and weekends.

Qualifications

• QMAP certification required for the position.
• Experience in assisted living/memory care is preferred.
• Reliable, compassionate caregiver with good communication.

Responsibilities

• Respond to resident call lights and assist with needs.
• Follow service plans and assist residents with activities of daily living.
• Escort residents to activities, dining, and other events.
• Deliver meals to individual apartments as needed.
• Clean residents' rooms and common areas.
• Provide laundry and linen services per the service plan.
• Document changes in residents' status and report to the Charge Nurse.
